RN, Residential Rehabilitation Treatment Program (RRTP) Transitions Coordinator

Statement of Major Duties & Responsibilities Coordinates treatment, facilitating communication and partnerships with team members, internal and external stakeholders to facilitate outreach, access, transitions, discharge planning and other services of benefit to RRTP Residents. Facilitates and participates in the screening and admissions processes. Utilizes Bed Management Solution (BMS) to track and coordinate Veteran admissions to SUD RRTP. Provides patient continuity of care from admission to discharge from Substance Use Disorder Residential Rehabilitation Treatment Programs (SUD RRTP), including provision of medication management, relapse prevention education, counseling, case management, and discharge planning. Documents patient interactions in the medical record. Ensures access barriers upon program admission and discharge are reduced or eliminated. Counsels and educates the patient and family on issues related to substance abuse disorders, mental illness, and disease prevention. Utilizes Measurement Based Care to inform treatment decisions in collaboration with the Resident and his/her case manager. Assists with discharge and transition planning, including arranging post-discharge mental health/substance use disorder appointments, aftercare plans and follow-up referrals in collaboration with the Resident and his/her case manager. Conducts psychoeducation in individual and group formats with focus on substance use disorders, mental health issues, coping and problem-solving skills, stress management and relaxation skills, developing recovery goals, health education (HIV/AIDs, safe sex practices, Opioid Education and Naloxone Distribution (OEND), Medication-Assisted Treatment (MAT), etc.). Provides nursing services across levels of care and to residential patients in SUD RRTP. Conducts initial nursing assessments for patients entering RRTP Provides medical management in collaboration with attending physician Reviews medication recommendations with appropriate supply of medications, including naloxone if clinically indicated. Assesses and identifies health-related illness using nursing assessments which follow national standards for nursing assessments and local VA mental health nursing assessment policies. Creates and maintains safe, therapeutic, recovery-oriented, trauma-informed care in collaboration with Residents and other healthcare providers. Utilizes critical thinking and sound judgment in the execution of duties and responsibilities. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Experience:
A minimum of 2 years of mental health experience for applicants with a BSN or a minimum of 5 years of mental health experience, including treatment coordination for patients with substance use disorders is strongly preferred. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Direct patient care work involves considerable walking and standing (up to 12 hours/day), pushing, pulling, bending, and heavy lifting (45 pounds and over). The incumbent accepts assignment on any tour of duty and in whatever area services are required. In-direct patient care work is mostly sedentary. Typically the employee sits comfortably to complete the work. However, there may be some walking, standing, bending and carrying of light items such as patient records and supplies

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
