Social Worker (Contract Nursing Home Coordinator)
The Social Worker will serve as the Coordinator of the Contract Nursing Home program (CNH), the Contract Adult Day Health Care program (ADHC) and State Home program.
The Social Worker is capable of practicing at the advanced level and has specialized knowledge of practice typically related to the population served within the CNH programs.
The Coordinator should have at least 2 years' experience in an area of specialized social work practice and licensed or hold certification in a state at the advanced practice level.
Other duties includes:
o The Coordinator is responsible for the CNH annual budget of approximately 8 million dollars.
o The Coordinator is delegated the oversight resposibility and authority for the planning, direct staff and coordination of all elements which constitute the CNH program.
o The Coordinator is independently responsible for assuring that Central Office Geriatrics and Extended Care program standards and policies are adhered to in a timely and effective manner for the CNH and ADHC programs.
o The Coordinator will need a high and varied level of professional expertise and collaboration to develop, revise and/or apply policies and standards which determine the scope of the combined programs and the supervision of duties of the program staff.
