Advanced Practice Nurse with Rx (Geriatrics and Extended Care)
o Provides leadership in managing/coordinating care for residents and families o Completes initial assessments and reassessments of resident's condition o Responds to treatment and other clinical findings o Initiates follow-up as needed and documents using clear, concise and objective data o Investigates complex resident care problems and promptly seeks consultation as needed o Develops and implements plan of care to meet individual resident and population specific needs, including self-care and treatment measures o Modifies approaches based on knowledge of human behavior, motivations, cultural considerations, age-related issues, and reactions to situations o Initiates and/or recommend laboratory tests, diagnostic studies and/or specialty referrals based on protocols, clinical practice guidelines and/or standards of care.
o Performs all other duties as assigned.
