Program Description Haven's Open Arms, LLC is a residential program located in Columbia County Florida, for abused and/or neglected adolescent girls and boys under the care of the Department of Children and Families.
Job Responsibilities 1) Interview youth via phone/Zoom/in-person for potential placement in the home, when needed 2) Intake/Admission and orientation of new youth 3) Create child case files and ensure all documentation is received from the case workers and all intake paperwork is completed and signed, 4) Assist with Conflict Resolution with youth behavior issues; Review incident reports and address issues in monthly meetings 5) Transporting of youth when needed 6) Review medication logs to ensure correct documentation 7) Conduct Independent living skills training 8) Hold monthly meetings with youth/staff to discuss safety issues, complaints and concerns 9) Supervise house parents & assist them as needed 10) Review all logbooks including online CareSoft documentation to ensure completion and updated 11) Create monthly Activity Calendars; Plan bi-weekly family outings 12) Correspond with all doctors, dentists, pharmacy, and counseling offices for all youth & update CareSoft 13) Ensure all doctor, dentist, counseling, and other pertinent appointments are up to date for all youth 14) Create monthly progress reports; create and update individual service plans 15) Other reasonable duties asked.
Requirements:
These employees shall be at least 21 years of age; have a bachelor's degree in a human service field or equivalent and three to five years of experience working with at risk youth; must demonstrate proficiency in Microsoft Office, Communication, and the ability to work well with youth.
Must have valid driver's license.
